In June 1823 he was transferred to the Colonial Medical Department as an assistant surgeon and posted to the Sydney Hospital. In about 1833 he opened a private practice at Cumberland Place which he ran in addition to his duties at the hospital. In 1845 he was elected to the Medical Board and became its president in 1852. His son David Scott Mitchell was a major benefactor to the State Library of NSW.
