The Hawkesbury Benevolent Society and Hospital at Windsor was established on 31 December 1818 by the residents of Hawkesbury at a meeting presided by William Cox. Its aim was to assist the infirm and old amongst the free and emancipated colonist of the district.

General Lachlan Macquarie granted the society land for cattle and in 1819 a herd was acquired which became the society's chief source of income for forty-five years. Other sources of income were donations, subscriptions, church collections and fines from the Police Offices. The society generally supplied food and clothing for the poor. In 1832 an asylum for the poor was established in a house and in 1836 a building was erected for that purpose.

In 1846 the society acquired the premises of the convict Hospital at Windsor. The society could now provide a refuge for the poor and hospitalization for the sick poor on the one premises. The society also dispensed charity in the form of food, clothing, shelter and medical treatment.

The society was called the Hawkesbury Benevolent Society and Asylum. In 1903 the society changed its name to the Hawkesbury Benevolent Society and Hospital. In 1936 the Hospitals Commission of New South Wales acquired supervision of the society as a public hospital under the Public Hospitals Act 1929.
